Understanding Chronic Condition Special Needs Plans

A Chronic Condition Special Needs Plan (C-SNP) is a type of Medicare Advantage (Part C) plan designed specifically for people who have certain severe or disabling chronic conditions. These plans are offered by private insurance companies approved by Medicare and must follow all Medicare rules and coverage requirements.


C-SNPs tailor benefits, provider networks, and prescription drug formularies to better support members living with qualifying chronic health conditions.

How a C-SNP Works

Like all Medicare Advantage Plans, a C-SNP:


  • Provides Medicare Part A (Hospital Insurance) and Part B (Medical Insurance) coverage
  • Sometimes includes Medicare Part D (prescription drug coverage)
  • Must cover all medically necessary services that Original Medicare covers
  • May offer additional benefits not covered by Original Medicare


Because C-SNPs are specialized plans, they focus on coordinating care for members with specific chronic conditions. Members generally must use doctors and providers within the plan’s network (except in emergencies), and prior authorization may be required for certain services.

Who is Eligible for a C-SNP?

To enroll in a C-SNP, you must:


  • Be entitled to Medicare Part A
  • Be enrolled in Medicare Part B
  • Live in the plan’s service area
  • Have one or more of the chronic conditions the C-SNP serves, as verified through medical documentation


Examples of chronic conditions covered by different C-SNPs may include (but are not limited to):

  • End-stage renal disease (ESRD)

What Makes a C-SNP Different?

C-SNPs are structured to provide more focused and coordinated care than standard Medicare Advantage plans. Features may include:

  • Disease-specific prescription drug coverage
  • Specialized programs to support medication adherence and lifestyle management


Plans must follow Medicare guidelines and ensure that coverage meets federal requirements.

Costs & Coverage

Costs vary by plan and may include:


  • A monthly premium (in addition to your Part B premium)
  • Copayments or coinsurance for services
  • Deductibles for medical and/or prescription drug coverage


Medicare Advantage Plans, including C-SNPs, have a yearly limit on out-of-pocket costs for covered Part A and Part B services . Once you reach that limit, you generally pay nothing for covered services for the rest of the year.
